General Information & Enrollment

MyDCHC Portal provides secure, online access to your personal medical information. MyDCHC Portal will allow patients to:

  • Send a secure electronic message to their healthcare provider
  • Request an appointment
  • Request a prescription renewal
  • View hospital and clinic notes
  • View radiology results
  • View lab results and vitals
  • Access healthcare resources
  • View and print medication lists
  • Update demographic information
  • View a family member’s medical record (authorization required)

When registering for services at Davis County Hospital & Clinics, if you choose to provide a valid email address, a secure email will be sent that contains an invitation link and login information.

Open the email and click on the link that says “Accept Invitation to MyDCHC Portal.” This will direct you to the MyDCHC Portal account set up page. You will be asked to provide information to verify your identity and set a password for your account.

If you do not see your invitation in your email’s inbox, please check your spam or junk mail folders in your email account. If you still cannot find it, please call the Davis County Hospital & Clinics at 641-664-7135.

If you are age 12 years and over, you may give authorization to allow a family member to view your information by completing an authorization form. Authorization forms can be found at one of our registration desks. In addition, individuals who have Legal Guardianship or an active Durable Power of Attorney for a person can request proxy access. An authorization form will need to be completed for each proxy access request. Proof of legal guardianship and Durable Power of Attorney is required for authorization. Questions regarding proxy access should be directed to the Health Information Management department at 641-664-7135.

PLEASE NOTE: Individuals will require a portal account to be created for themselves, before accessing the patient’s info they have been authorized to access.  Not a patient at DCHC? No problem. Our registration team can set-up you up with an account.

You will only have medical information in your portal account if you have been seen at the hospital or clinics since April 15, 2019. Previous medical information is “pre-loaded” into your health record prior to your visit, but that information is not loaded into your portal. Only information that has been recorded in our new electronic health record will be included in MyDCHC Portal. Additionally, to protect your privacy, not all medical information from your health record is released to the patient portal.

It is common practice in health care to utilize the metric system. Unfortunately, we are unable to convert this data for viewing in MyDCHC Portal. If you would like to convert the values, please utilize one of the many conversion tools that can be found online.

MyDCHC Portal Features

Laboratory and Radiology test results may be delayed in appearing on MyDCHC Portal so your Physician or Nurse Practitioner can review the results and may contact you directly if needed.

Test results of a sensitive nature are not released to MyDCHC Portal.

Yes, MyDCHC Portal will allow patients to make online prescriptions renewal requests.

Yes. You can request an appointment by visiting MyDCHC Portal and clicking on “Request an Appointment.” You will enter in your desired appointment time and will receive a secure electronic message or phone call back to confirm your appointment.

When you select a provider to contact in MyDCHC Portal, the message will be sent through your electronic medical record to that provider’s clinic.

You will receive a response within one business day (Monday through Friday, 8 a.m. to 5 p.m.).

Yes. Please call us at 641-664-2145 instead of using online messaging for any of the following purposes:

  • You need to see a provider today
  • You need a prescription filled today
  • You need to cancel or reschedule an appointment in the next 24 hours

No. MyDCHC Portal should not be used to communicate an emergency. If you have a true emergency, please call 911.

Technical Questions

Passwords can be reset without having to contact DCHC. Please visit the login page and click on the Forgot Password link to reset your password.

Davis County Hospital & Clinics takes great care to ensure your health information is kept private and secure. The authorization process is controlled through a secure invitation process, personal IDs, and passwords.

Cerner is a Kansas City based company that provides Davis County Hospital & Clinics with the Electronic Health Record used by our providers and nurses during your visit or hospital stay. This electronic system has replaced the “paper charting” system of the past.

There are multiple ways to access your MyDCHC portal:

  2. On a mobile phone via an application:
    a. Connect Your Health Management Apps: The applications listed below are already connected to our health record. Check back here as applications will be regularly added as they are connected.
    Apple Health RecordsMyDCHC Portal users can now access important health data through the iPhone's built-in Health app. With Apple Health, information on immunizations, lab results, medications and vitals is stored alongside data about your activity, sleep, nutrition and more.

    OneRecord
    After downloading the OneRecord app, you can seamlessly locate your records using your patient portal accounts and our technology. Search for your doctor, practice, or health system and then consolidate all of your health records in one place.
