Davis County Medical Associates is hosting a FREE Sports Physicals Clinic on Thursday, July 20th from 4:00pm – 6:45pm at Davis County Hospital.

Registration deadline is July 12th. Space is limited to 112 students.

If your child will be entering 7th or 12th grade, please read below.

As mandated by the Iowa Department of Public Health, every student entering 7th grade is required to show proof of Tdap (Tetanus, diphtheria, and pertussis) vaccination prior to the first day of school.
In addition to this and beginning with the 2017-18 school year, and all future school years:
- All students entering, advancing or transferring into 7th grade and born after September 15, 2004, will need proof of one dose of meningitis vaccine.
- All students entering, advancing, or transferring into 12th grade and born after September 15, 1999, will need proof of two doses of meningitis vaccine; or 1 dose if received after the age of 16.
*If your child will be entering 7th or 12th grade, the above mentioned vaccines will be available during the clinic. If you choose to have your child vaccinated during the clinic, you will be asked to provide your insurance information when you call to pre-register. The hospital staff will contact your insurance company and if any co-pay is required for the vaccinations, you will be notified prior to the clinic date so you can take care of the co-pay at time of registration.
