You can request an appointment online by visiting MyDCHC Portal and clicking on “Request an Appointment.” You will enter in your desired appointment time and will receive a secure electronic message or phone call back to confirm your appointment.
Requests made during non-business hours, weekends or holidays, will be returned the first business day following the request.
