Immunization Clinics
2nd Wednesday of the month 8:30-11:30 AM &
Last Wednesday of the month 3:00-6:00 PM.
Appointments are not required.
Available Immunizations for Children (birth-18 years old):
- Polio
- Measles, Mumps, Rubella
- Tetanus, Diphtheria, Pertussis
- Hepatitis B
- HIB
- Rotavirus
- Varicella (chicken pox)
- HPV (gardisil)
- Meningitis
- Prevnar
- Seasonal influenza for adults and children
Adult immunizations are available upon request.

Home Care Aide Service
Personal care assistance including bathing, dressing, hair care, nail care, & other activities of daily living.
Homemaker Service
Assistance with general light housekeeping, errands, meal preparation, grocery shopping, & laundry.

Fee for all services are based on a sliding fee scale. A qualified Registered Nurse will assist in determining fee for service and development of an individualized plan of care during their initial visit.
