Davis County Hospital Foundation Annual Golf Tournament Fundraiser

The Davis County Hospital Foundation is excited to invite you to participate in our Annual Davis County Hospital Foundation Golf Tournament. This golf outing will take place at the Bloomfield Country Club on Sunday, September 10.
We would like you to be a part of this annual event and join teams from around the area who participated in the golf tournament in past years.
The Davis County Hospital Foundation is a non-profit organization and is classified by the Internal Revenue Service as a 501(c)3 organization.
The golf tournament raises funds through individuals playing in the tournament, sponsors, and raffling donated auction items.

$65 per person
Golfers registration includes:
Player fees, 18 holes of golf, 3 meals, a fabulous gift from the Foundation, raffle ticket, mulligan, and free drinks. (cart rental not included)
